At Panacea Massage Therapy, safeguarding personal information is a priority. We are committed to responsibly collecting, using, and disclosing personal information solely to the extent necessary for the services we provide. This document outlines our privacy practices and policies.

Who We Are

Panacea Massage Therapy is a multidisciplinary healthcare clinic located in Toronto, Ontario. Our team of registered healthcare professionals is dedicated to delivering the highest standard of care to our clients.

Why We Collect Personal Health Information 

We collect, use, and disclose personal information to provide the best possible care to our clients.

 

  1. Primary Purpose: Personal health information is collected to deliver appropriate treatment. This includes details about a client’s health history, family medical history, physical condition, functionality, and social circumstances. This information helps us assess health needs, offer options, and provide tailored care.

  2. Secondary Purpose: Establishing a health and social baseline helps us monitor and identify changes over time as part of ongoing care.

Financial Information

We collect financial information, such as credit card details (number, expiration date, and CCV), through our clinic management and scheduling software, JANE. Payments are processed securely via PCI-compliant and SSL-certified provider Stripe.

Financial information is used for:

  • Scheduling future appointments.

  • Enforcing our cancellation policy for missed appointments or cancellations made with less than 12 hours’ notice.

Jane stores data on secure, SOC 2-audited servers. All data is encrypted using 128-bit encryption during transfer and 256-bit encryption at rest, ensuring robust security comparable to banking standards.

Disclosure of Personal Information 

Personal information may be disclosed only with the client’s written consent. For instance, copies of a client’s file may be shared with lawyers or insurance companies upon receiving a formal request accompanied by consent.

Healthcare Information Custody & Storage

As required by law, all healthcare clinics must designate a Healthcare Information Custodian (HIC) responsible for safeguarding clients' personal information.

At Panacea Massage Therapy, Ann Hoang serves as the acting HIC, ensuring the proper storage and protection of all client records.

Accessing Your Records

Clients maintain ownership of their Personal Health Information (PHI). Upon written request, clients can:

  • Obtain copies of their records.

  • Request their records be sent to another healthcare provider.

By law, PHI must be retained for at least 10 years from the date of the client’s last appointment or 10 years after the client’s 18th birthday, whichever is later.

Destruction of Records

When paper records are digitized into the JaneApp system, the original documents containing personal information are securely shredded.

Legal Compliance

This policy complies with the Personal Health Information Protection Act, 2004 (PHIPA). The act contains additional exceptions and provisions not detailed here due to their complexity.
